Financial Policy and Insurance Payments
Charges for office visits, procedures, and treatments are due at the time of service. Co-payments and deductible will be collected when you check in for your appointment. Payment for any uncovered services is expected upon check out — we accept cash, check or credit card.
Insurance
We participate in most insurance and managed care plans. As designated providers for these carriers, we will file for payment of covered charges with your insurance carrier. In order for us to properly file charges, we will require a photocopy of your insurance card and a photo ID at the time that you check in for your appointment. If you have supplemental carrier coverage, we will need a photo copy of that insurance card as well — please remember to bring these items to each appointment.
If you are covered by managed care insurance, we will need to verify that you are currently eligible for benefits before the doctor sees you. The majority of these organizations require prior authorization for each visit and the patient is responsible for acquiring office visit authorizations.
Please remember: Regardless of the type of insurance or the party you feel is responsible for your bill, our agreement for treatment is with you. We will ultimately look only to you for payment of your account.
